Abstract :The vast majority of all chest traumas are blunt traumas. In blunt traumas, serious intrathoracic intrapleural complications such as pneumothorax, hemothorax, hemopneumothorax may be observed whether or not accompanied by rib fractures, and in some cases, thoracic wall hemorrhages may be observed rarely at a level that severely impairs vital values. In our case, we presented a patient who developed secondary to multiple rib fractures, no intrapleural complications were observed, vital values were severely impaired due to diffuse bleeding in the serratus anterior muscle, which is a very rare complication, and we performed rib fixation with a non-classical method (intrathoracic extrapleural approach).Keywords : intrathoracic extrapleural, rib fixation, rib fracture
