Abstract :Diş hekimlerinin karşılaştığı kontrol altına alınması en zor problemlerden biri odontojenik enfeksiyonlardır. Çürük, periodontal hastalık ve pulpitis gibi başlangıç enfeksiyonları dişlerden alveoler kemiğe ve yüzün, oral kavitenin, baş ve boynun derin dokularına yayılım gösterebilirler. Odontojenik enfeksiyonlar; minimal tedavi gerektiren, iyi lokalize, düşük dereceli enfeksi- yonlardan, hayati tehlike arz eden, derin fasiyal yapıla- rın enfeksiyonuna kadar farklılık gösterebilir. Her ne kadar odontojenik enfeksiyonların büyük çoğunluğu minor cerrahi müdahalelerle ve antibiyotik uygulan- masını da içeren destekleyici tedavilerle kolayca kontrol altına alınabiliyorsa da; hekim her zaman bu enfeksiyonların kısa süre içinde ciddi ve hayati tehlike arz edebilecek duruma gelebileceğini aklından çıkar- mamalıdır. Çenelerin osteomiyeliti bu gibi inatçı, tedavisi zor enfeksiyonların bir sonucu olarak ortaya çıkabilir. Tıp ve diş hekimliği alanlarındaki gelişmeler ve antibiyotiklerin devreye girmesiyle birlikte tanı ve tedavi süreci kolaylaşmış ve mortalite oranları azalmış olsa da osteomiyelit hala oldukça ciddi bir hastalıktır. Bu derlemede çenelerde görülen osteomiyelit tipleri ve tedavilerinden bahsedilecektir. Anahtar Kelimeler: osteomiyelit, kronik reküren multifocal osteomiyelit, kemik hastalıkları OSTEOMYELİTİS OF THE JAWS Odontogenic infections are one of the most difficult and hard to manage problems that dentists encounter. Early infections such as caries, periodontal diseases and pulpitis may spread from the teeth to the alveolar bone and to deeper tissues of oral cavity, head and neck. Odontogenic infections could differ from localized, low-grade infections which require little treatment to infections of deep facial structures which may be life threatening. Although a majority of odontogenic infections may easily be controlled with minor surgical interventions together with antibiotic applications; clinicians should always keep in mind that these infections may become very serious and life threatening in a short amount of time. Osteomyelitis of the jaws may occur as a consequence of such persistent, hard to treat infections. Although the diagnostic and treatment processes of osteomyelitis have become easier and the mortality rates dropped down with the advancements in dentistry and medicine and antibiotics coming into play in the treatment of infections; osteomyelitis is stil a very serious disease.
