PERİODONTAL HASTALIKLI BİREYLERDE PRO İNFLAMATUAR SİTOKİN GEN POLİMORFİZMLERİNİN DEĞERLENDİRİLMESİ

Abstract :Amaç: Periodontal hastalıklar, pro-inflamatuar sitokin yanıtındaki artışla gözlenen epigenetik karakterli kronik inflamatuar patolojilerdir. Bu çalişmada, periodontal hastalıklı bireylerde pro-inflamatuar sitokin insert ignore into journalissuearticles values(interlökin insert ignore into journalissuearticles values(IL);-1β, tümör nekrozis faktör-alfa insert ignore into journalissuearticles values(TNF-α); ve IL-6); gen polimorfizmlerini değerlendirmeyi amaçladık. Yöntemler: Bu çalışmaya periodontitisli insert ignore into journalissuearticles values(n = 25);, gingivitisli insert ignore into journalissuearticles values(n = 22); ve periodontal olarak sağlıklı kontroller insert ignore into journalissuearticles values(n = 18); olmak üzere toplam 65 birey dahil edildi. Periodontal parametreler insert ignore into journalissuearticles values(plak indeksi, gingival indeks, sondalama cebi derinliği, sondalamada kanama yüzdesi ve klinik tutunma seviyesi); kaydedildi ve açlık venöz kan örnekleri alındı. TNF-α-308, -238, IL-1β -511, +3953 ve IL-6 -174 ve -572 polimorfizmlerinin genotipleri ve alel frekansları polimeraz zincir reaksiyonu-kısıtlı parça uzunluğu polimorfizmi tekniği kullanılarak yapıldı. Bulgular: IL-1β +3953, IL-6 -174 ve TNF-α-238 heterozigot genotipleri, klinik periodontal parametreler ve de mografik özellikler periodontitisli bireylerde artış gösterdi. IL-1β +3953 insert ignore into journalissuearticles values(risk alel 2 insert ignore into journalissuearticles values(A alel); insert ignore into journalissuearticles values(GA genotipi); periodontitis riskini arttırırken, IL-6 -174 insert ignore into journalissuearticles values(risk aleli 2 insert ignore into journalissuearticles values(G/T alleli); insert ignore into journalissuearticles values(CG/CT genotipleri); gingivitis ile ilişkiliydi. Sonuç: Bulgularımız, pro-inflamatuar sitokin gen polimorfizmlerinin gingivitis ve periodontitis patogenezinde etkili olabileceğine dair önemli kanıtlar sunmaktadır. Periodontal hastalıklarla ilişkili sitokin yanıtının kontrolü pek çok enfeksiyöz-inflamatuar hastalıkla ilişkili mortalite ve morbidite oranlarının azaltarak halk sağlığının arttırılmasına yönelik bir adım oluşturabilir. Anahtar Kelimeler: İnterlökin 6, interlökin-1 beta, diş eti iltihabı, periodontit, polimorfizm, tümör nekrozis faktör alfa ABSTRACT Objective: Periodontal diseases are chronic inflammatory pathologies including in pro-inflammatory cytokine-mediated pathogenesis and epigenetic characteristics. We aimed to evaluate pro-inflammatory cytokine [interleukin insert ignore into journalissuearticles values(IL);-1β, tumour necrosis factor-alpha insert ignore into journalissuearticles values(TNF-α);, and IL-6] gene polymorphisms in patients with periodontal disease. Methods: Patients with periodontitis insert ignore into journalissuearticles values(n = 25);, gingivitis insert ignore into journalissuearticles values(n = 22);, and periodontally healthy controls insert ignore into journalissuearticles values(n = 18); were enrolled in this study. The periodontal parameters insert ignore into journalissuearticles values(plaque index, gingival index, probing pocket depth, percent age of bleding on probing, and clinical attachment level); were recorded, and fasting venous blood samples were obtained. The genotypes and allele frequencies of TNFα -308, -238, IL-1β -511, +3953, IL-6 -174, -572 polymorphisms were performed using polymerase chain reaction–restriction fragment length polymorphism technique. Results: Patients with periodontitis had the highest IL-1β +3953, IL-6 -174 ve TNF-α -238 heterozygot genotypes, clinical periodontal parameters and demographic characteristics. IL-1β +3953 [the risk allele 2 insert ignore into journalissuearticles values(A allele); insert ignore into journalissuearticles values(GA genotype);] led to an increased risk of periodontitis, IL-6 -174 [the risk allele 2 insert ignore into journalissuearticles values(G/T allele); insert ignore into journalissuearticles values(CG/CT genotypes);] was associated with gingivitis. Conclusion: Our findings provide important evidence that pro-inflammatory cytokine gene polymorphisms may be involved in the pathogenesis of gingivitis and periodontitis. Control of the cytokine response associated with periodontal diseases may constitute a step towards improving public health by reducing mortality and morbidity rates associated with many infectious-inflammatory diseases.
