Abstract :Change is a difficult process for all sectors. For organisations that are in constant change, it is very important to cope with change in processes such as maintaining competitive advantage in the market they are in, adopting new technologies, improving their quality and keeping the organisation dynamic in the transition to this change. Management of the change process in primary health care services is of great importance. Because the appropriate and systematic execution of these changes is directly related to both the improvement of organisational performance and the effectiveness of health services. However, the change process is challenging and creates serious difficulties for primary health care managers. In the face of population ageing and increasing costs, one of the most effective ways to overcome the difficulties in meeting health needs from the provision of integrated health services to disease management in today`s health systems is to have a strong primary health care infrastructure.
