Abstract :Skeletal muscle is an extremely uncommon site for metastases. We report a 47 year old male patient with a previous history of left radical nephrectomy due to renal cell carcinoma. Initial PET/CT scan in in March 2008 showed a slightly hypermetabolic focus in the right infraspinatus muscle consistent with solitary intramuscular metastasis. A repeat PET/CT scan in July 2009 revealed metabolic progression of right intraspinatus metastasis as well as other foci in right rectus abdominis, piriformis and left vastus medialis muscles. PET/CT is useful for the detection of intramuscular metastases of renal cell carcinoma. Increased focal FDG uptake in muscle groups should be carefully evaluated when reading PET scans
