Abstract :Bilateral ovarian cysts of 5 cm diameters each were detected in the fetus of a 30-year-old gravida 1 pregnant woman during 30 weeks of gestation. Postnatal ultrasound and MR scans revealed multicystic lesions in both ovaries R:10 cm, L: 7cm without any solid component, and displacement of intestine and kidneys. At operation, after performing bilateral wedge resection, drilling procedure was done. The histopathology results reported multiple aluteinized follicular cysts without any primordial follicles. Our case presents difficulty in terms of treatment choices particularly because of bilateral disease without any normal ovarian tissue
