Abstract :Cardiovascular diseases, which are among the leading chronic diseases, are the most important cause of morbidity and mortality all over the world and in our country. The aim of primary prevention of cardiovascular disease is the prevention of disease in individuals who have had risk factors but no evidence of disease yet. The aim of secondary prevention is to prevent progression of the disease in individuals who already have diagnosis of coronary or other atherosclerotic vascular disease. In many randomized investigations about nurse case management models in secondary prevention according to routine care, significant improvement in risk factors, exercise tolerance, blood glucose control, appropriate usage of drugs and decrease in cardiac events and mortality, regression of coronary atherosclerosis and improvement in the perception of the patient’s health were found. This article includes the nurse-led secondary prevention recommendations for an effective prevention program in the health promotion of program in health promotion with coronary artery disease patients
