Abstract :Today, the most widely used scale for the assessment of neurological status is the Glasgow Coma Scale GCS . The limitations of the GCS include verbal response or difficulty in aphasic patients intubated. Full Outline of the unresponsiveness FOUR is a new coma scale that is recently proposed and not dependent on verbal response. This article will examine consciousness, levels of consciousness and a new Four Score scale that can be used to assess the level of awareness which may be of particular interest to intensive care and emergency nurses working in critical units
