Abstract :ÖZET Üst ekstremite cerrahisi için genel anestezi veya rejyonel anestezi kullanılabilir, ancak ciddi akciğer hastalıkları olan hastalarda genel anestezi, mortalite ve morbidite artışıyla ilişkilidir. Bu olgu sunumunda, bilateral büllöz akciğer hastalığı olan ve üst ekstremite cerrahisi geçirecek bir hastanın ultrasound rehberliğinde yapılan infraklaviküler blokajının sunulması amaçlandı. Spontan pnömotoraks hikayesi bulunulan, ciddi büllöz akciğer hastası 50 yaşında erkek hasta, sağ olecranon fraktürü nedeniyle hastanemize başvurdu. Ultrasound rehberliğinde ve sinir stimülatörü kullanılarak infraklaviküler blok uygulaması planlandı ve uygulandı. Herhangi bir komplikasyon olmadan cerrahi tamamlandı ve hasta servise transfer edildi. Postoperatif analjezik, ameliyattan sonraki 8. Saate kadar gerekli olmadı. Sonuç olarak, rejyonel anestezi ciddi pulmoner komorbiditeleri olan hastalarda, genel anestezi ile karşılaştırıldığında daha güvenli bir teknik olabilmektedir. Sinir stimülasyonu ile birlikte ultrasound kullanımı kombine edilerek, periferik sinir blokları sırasında oluşabilecek pnömotoraks ve intranöral enjeksiyon gibi komplikasyonların minimize edilebileceği kanısındayız. Anahtar Sözcu¨kler: İnfraklaviküler blok; Pnömotoraks; Ultrasonografi; Olekranon kırığı ABSTRACT The general anesthesia or regional anesthesia can be used for the upper extremity surgery, though the general anesthesia is related to increasing mortality and morbidity in patients with severe pulmonary diseases. In this case report, we aimed to present the ultrasound-guided infraclavicular block technique due to upper extremity surgery patient who have bilateral bullous lung disease. 50-year-old male patient who had severe bilateral bullous lung and with a history of spontaneous pneumothorax was admitted to our hospital because of the right olecranon fracture. Ultrasound-guided nerve block and simultaneous nerve stimulation was planned to perform the right infraclavicular block. Surgery was performed without any complications and patient was transferred to ward. Postoperative analgesic was not necessary until the eight hours after surgery. As a result, regional anesthesia could safe technique compared to general anesthesia particularly patients who have severe pulmonary comorbidities. We also think that the combination of ultrasound and nerve stimulation are minimized the complications such as pneumothorax and intraneural injection during peripheral nerve block interventions.
