Abstract :Petröz apeks, önemli yapılara yakınlığı sebebiyle temporal kemiğin oldukça kompleks bir bölgesidir. Petröz apeksin kolesteatomları nadir görülür ve genellikle doğumsaldır. Bu lezyonlar teşhis konmadan önce genellikle labirent ve/veya fasiyal kanala yayılırlar. Bununla birlikte klinik bulgular nonspesifiktir ve teşhis bu sebeple gecikebilmektedir. Petröz apeks kolesteatom cerrahisi oldukça zordur. Lezyonun temizlenmesi için çeşitli cerrahi yaklaşım teknikleri bildirilmiştir. Bu makalede geç teşhis konan ve modifiye transkoklear yaklaşımla tedavi edilen bir doğumsal petröz apeks kolesteatomu olgusu sunuldu.
