İlaç Hatalarına Yönelik Hemşirelerin Bilgi ve Tutumlarının İncelenmesi

Abstract :İlaç Hatalarına Yönelik Hemşirelerin Bilgi ve Tutumlarının İncelenmesi   Özet (≤250 KELİME) Amaç: Bu çalışma; hemşirelerin ilaç uygulama ve ilaç hatalarını raporlama süreçlerine  ilişkin bilgi ve tutumlarının ve bunları etkileyen faktörlerin belirlenmesi amacıyla  yapılmıştır. Gereç ve Yöntem:Çalışma; 956 hemşirenin katılımıyla 2016 yılı Temmuz  ayında gerçekleştirilmiştir. Verilerin toplanmasında; Sosyodemografik Veri Formu ile İlaç  Hatalarına Yönelik Hemşire Bilgi ve Tutum Ölçeği kullanılmıştır. Bulgular: Hemşirelerin   ilaç uygulama sürecine ilişkin ortalama doğru cevap verme yüzdesi 45.3’tür. Doğru   cevap verme ile kurumda çalışma süreleri arasında negatif (r=-.064, p=.049) anlamlı  ilişki saptanmıştır. Hemşirelerin %79.7’si hata raporlarını performans ölçütü görmekte,  %21.1’si yaptığı ilaç hatasını bildirmemekte, benzer oranlarda diğer hemşire, hekim  ve eczacının hatasını raporlamamakta, %60.1’i hataların paylaşılmasına yönelik kurum  politikasını bilmemekte, %45.7’si kısaltmaları kullanmakta, %41.3’ü yüksek riskli ilaçları  uygularken her zaman çift kontrol yapmamakta, %83.1’i sözel order prosedürüne uygun  davranmakta, %90’ından fazlası uygulama öncesi hasta alerji ve kimlik bilgilerini kontrol  etmektedir.   Sonuç: Bu çalışmada; hemşirelerin ilaç uygulama ve hataları raporlama  süreçlerine yönelik olumlu ve olumsuz bilgi ve tutumlarının olduğu belirlenmiştir.  Hastanelerde etkin ve güvenli ilaç yönetim sisteminin yanı sıra hata raporlama sisteminin,  cezalandırıcı olmayan kurum kültürünün, ilaç güvenliğine yönelik hizmetiçi eğitimlerin,  kurumların ilaç uygulama süreçlerini ve hatalarını periyodik değerlendirmesinin ilaç  güvenliğinin geliştirilmesinde katkı sağlayacağı sonucuna varılmıştır. Anahtar Kelimeler: (3-5 Kelime): Hemşire, ilaç hataları, bilgi, tutum   Investigation of Knowledge and Attitude of Nurses Towards Medication Errors   Abstract (≤250 WORDS) Objective: This study was conducted to determine the knowledge and attitudes of  nurses towards medication administration and reporting of medication errors and the  factors affecting them. Material and Methods: The research was carried out in July,  2016 with the participation of 956 nurses. Data were collected by using the Sociodemographic Data Form and Nurse Knowledge and Attitude Toward Medication Errors  Survey. Findings: The mean achievement ratio of the nurses about administration of  medication was 45.3%. A significant negative correlation was found (r=-.064, p=.049)  between the achievement and the duration of employment at the relevant institution.  Error reports were considered as a performance criterion by the 79.7% of nurses. Own  medication errors were not reported by 21.2% of the nurses and similar rate of the  nurses did not report the other nurses’, doctors’ and pharmacists’ errors. The institution’s  error sharing policy was not known by 60.1% of the nurses. 45.7% of the nurses used  abbreviations, 41.3% did not always apply double-checking when administering highrisk drugs and 83.1% of the nurses implemented the verbal order standart operation  procedure, more than 90% of the nurses checked the patient’s allergy and identity  information before the medication administration. Conclusion: In this study, positive  and negative knowledge and attitudes were determined to be present towards  medication administration and reporting of medication errors of the nurses. It was  concluded that hospitals should establish a policy, a non-punitive corporate culture and  an in-service training about the safe medication administration, periodical evaluation of  the medication administration process and reporting the medication errors in addition  to efficient and safe medication management system.
