Abstract :Neonatal cholestasis, theprolonged elevation of serum levels of conjugated bilirubin, is most frequently noted in the first months of life. Cholestasis may be due to infectious, genetic, metabolic, or undefined abnormalities giving rise either to mechanical obstruction of bile flow or to functional impairment of hepatic excretory function and bile secretion. Neonatal cholestasis remains a major diagnostic challenge despite increasing knowledge regarding its pathogenesis. Appropriate interpretation of imaging and pathologic studies requires a pediatric center familiar with the entities causing neonatal cholestasis. When liver failure or progressive hepatic dysfunction is likely to occur, early referral to a liver transplant center is recommended. Neonatal kolestazis, genellikle ilk bir ay içinde ortaya çıkan ve serumda direk bilirubin yükselmesi ile seyreden bir hastalıktır. Enfeksiyonlara, genetik ve metabolik nedenlere bağlı olabileceği gibi safra atılımını bozan mekanik veya fonksiyonel nedenlerle de gelişebilmektedir. Patogenezi hakkındaki bilgi artışına rağmen tanısında halen önemli ölçüde güçlük çekilmektedir. Neonatal kolestazisli hastaların değerlendirilmesi ve izle mi deneyimli Pediatri Kliniklerince yapılmalıdır. Karaciğer yetmezliği veya progressif karaciğer disfonksiyonu geliştiğinde hastanın bir karaciğer transplantasyon merkezine refere edil¬mesi gerekir.
