Abstract :Bu çalısmanın amacı, acil peripartum histerektomi (APH) insidansı, endikasyonları ve maternal özelliklerini saptamaktır.Diyarbakır Kadın Hastalıkları ve Dogumevi Hastanesinde Eylül 2005 ile Agustos 2008 tarihleri esnasında APH uygulanan 49 hasta retrospektif olarak incelendi. Üç yıl süresince gerçeklesen 63463 dogum arasından 49 hastaya (0.77/1000) obstetrik kanama nedeniyle histerektomi uygulandı. Acil peripartum histerektomi endikasyonları sıklık sırasına göre; uterin atoni (%49), uterin rüptür (%42.9) ve plasenta akreata (%8.2) olarak saptandı. Hastalarımızın ortalama yası 34.6±5.7 yıl, ortalama gravidaları ise 5.5±2.4 idi. Tedavi olarak, 31 hastaya (%63.3) subtotal histerektomi, 18\'ine (%36.7) ise total histerektomi uygulandı. Sekiz (%16.3) hastada bilateral hipogastrik arter ligasyonu, 7\'sine (%14.3) unilateral ooferektomi uygulandı. _ki hastamız kontrol edilemeyen kanama nedeniyle eksitus oldu. Sonuç olarak, uterin atoni ve uterin rüptür acil peripartum histerektominin en sık nedenleridir. Obstetrik kanamalarda acil ve uygun tedavi hasta hayatı için son derece önemlidir.
