Retrosternal Yapışıklıkların Önlenmesinde Bir Tavşan Modeli

Abstract :Primer median sternotomiyi takiben gelişen retrosternal-epikardiyal yapışıklıkların önlenmesinde, `Deksametazon-Ringer Laktat` kombinasyonuyla, `Hyalarunik Asid- Karboksimetil Sellüloz insert ignore into journalissuearticles values(HA- CMC); esaslı bioresorbable membran` kullanımının etkinliği değerlendirildi ve karşılaştırıldı. Bu amaçla, 10` ar tavşandan oluşan 3 grup oluşturuldu. Birinci grup insert ignore into journalissuearticles values(Grup I); kontrol, ikinci grup insert ignore into journalissuearticles values(Grup II); Deksametazon-Ringer Laktat, üçüncü grup insert ignore into journalissuearticles values(Grup III); HA-CMC grubundan oluşuyordu. Bu ajanlar, sternotomi ve perikardiyotomi sonrası epikardiyal sahaya topikal uygulanarak sonuçlar değerlendirildi. Grup I` in yapışıklık skor ortalaması, Grup II` nin yapışıklık skor ortalamasından anlamlı oranda yüksekti insert ignore into journalissuearticles values(p 0.05);. Grup I` in kronik enflamatuar hücre yanıtı, yapışıklık skoru ve yapışıklık yüzde ortalamaları Grup III` ten anlamlı ölçüde yüksekti insert ignore into journalissuearticles values(p 0.05);. Grup-II` nin akut enflama-tuar hücre yanıtı ve yapışıklık yüzde ortalamaları grup-III` ten anlamlı ölçüde yüksekti insert ignore into journalissuearticles values(p 0.05);. `Deksametazon-Ringer Laktat` ve `HA-CMC içeren bioresorbable membran` kullanımının, postoperatif epikardiyal ve retrosternal yapışıklıkları belirgin olarak azalttığı tespit edildi. HA-CMC içeren bioresorbable membran kullanımı, retrosternal yapışıklıkların engellenmesinde, Deksametazon-Ringer Laktat kullanımından daha etkili Prevention of Retrosternal-Epicardial Adhesion Formation in a Rabbit Model Retrosternal epicardial adhesions may cause significant injuries to the heart and great vessels in cases that require reoperation. We used a varity of agents to prevent the occurence of adhesions and observed different results. An experiment with 3 groups of rabbits with 10 rabbits in each group was prepared to investigate the roles of Dexamethazone-Ringer Lactate insert ignore into journalissuearticles values(DM-RL); combination and Hyaluronic Acid-Carboxylmethyl Cellulose insert ignore into journalissuearticles values(HA-CMC); in preventing adhesions. First group insert ignore into journalissuearticles values(Group I); was the control group, the second insert ignore into journalissuearticles values(Group II); was tested for DM-RL and the third group insert ignore into journalissuearticles values(Group III); was tested for HA-CMC. These agents were topically applied to the epicardial region after the sternotomy and pericardiotomy phases. The adhesion score was significantly higher in the first group than the adhesion score in the second group insert ignore into journalissuearticles values(p 0.05);. The second group had higher acute inflammatory cell response and adhesion percentage than the third group in statistical terms insert ignore into journalissuearticles values(p<0.05);. There was no statistically differences of fibrosis, adhesion score and cronic inflammatory cell response in these groups. We have found that the use of bioresorbable membrane containing DM-RL and HA-CMC clearly decreases the postoperative epicardial and retrosternal adhesions. The use of membrane containing HA-CMC is found to be more effective than the use of membrane containing DM-RL.
