Endotrakeal Aspirat Örneklerinden İzole Edilen Gram Negatif Etkenler

Abstract :Objective: Nosocomial infections are major cause of morbidity and mortality at intensive care units in both developing and developed countries. Endotracheal aspiration samples ETS are useful for diagnosis and setting treatment schemes as well as following the answers to the antibiotics. The aim of this study is evaluation of the microorganisms and their antimicrobial resistance isolated from ETS. Patients and Methods: Total number of 304 samples were included in this study which were sent to Clinical Microbiology Laboratory during the period of 2011-2012. All the data were evaluated retrospectively. Bacteriological studies were held conventionally. Automated system Vitek 2, France was used for identification and antimicrobial susceptibility. Results: Acinetobacter baumannii 42% , Pseudomonas aeruginosa 28% and Klebsiella pneumonia 10% were found to be the most common agents in 304 patient samples. High level of resistance to carbapenems ≥ 89% among A. baumannii isolates was striking. MIC50 and MIC90 levels of doripenem, imipenem and meropenem for these isolates were similar and were given as ≥ 8 μg/ml, ≥ 16 μg/ml, ≥ 16μg/ml, respectively. Resistance to carbapenems was also high among P. aeruginosa 80% isolates. MIC50 levels of doripenem, imipenem and meropenem for pseudomonas were calculated as ≥ 8 μg/ml, ≥ 8 μg/ml, ≥ 16μg/ml, respectively and MIC90 levels were ≥ 8 μg/ml, ≥ 16 μg/ml, ≥ 16μg/ml, respectively Conclusion: Contaminated environments of hospitals serve as sources for endemic resistant pathogens such as A. baumannii and P. aeruginosa. Methods used for isolation, identification of bacteria and determination of drug resistance are highly time consuming and expensive. Accurate, fast and cost-effective methods are needed to handle such microorganisms.
