Abstract :Theoreticians have made great efforts to define effective leadership, and have established many theories throughout history. They first sought the key for effective leadership in the characteristics of leaders, but then they realized that followers also play an important role in the leadership process. Transformational and transactional leadership are among the modern theories that involve followers in the leadership process. In transformational leadership, leaders change and direct the followers and their values, beliefs, and expectations in line with common objectives. In transactional leadership, leaders try to direct their followers using rewards and punishments. The effective leadership behaviors of nurses, the largest work force in health institutions, are of great importance for the quality and success of health services. Therefore, nurses should improve their leadership skills by reviewing the literature on leadership. Studies report that, although transformational leadership has more positive effects on clinical outcomes than transactional leadership, it is not successful in all circumstances. They also show that effective leadership can only be achieved by balancing these two types of leadership. In this regard, this study aimed to explain transformational and transactional leadership and follower, and analyze their effects on health services
