YURTDIŞI KAYNAKLI PLASMODİUM FALCİPARUM OLGUSU

Abstract :Tropikal ve subtropikal bölgelerde yaygın olarak saptanan sıtmanın ülkemizde genellikle P. vivax türü görülmektedir. Ancak yurt dışı seyahat öyküsü bulunanlarda P. falciparum sıtmasına da rast- lanmaktadır. Biz burada Sudan’da  P. falciparum’a bağlı sıtma şüphesi ile tedavi başlanılan, ve ta- kibi kliniğimizde yapılan bir olguyu sunduk. Otuz üç yaşında erkek hasta, halsizlik ve iştahsızlık şikayetleri ile polikliniğimize başvurdu. Öyküsünde Sudan’da çalıştığı öğrenildi. Bir hafta önce ateş yüksekliği, titreme, terleme, iştahsızlık ve halsizlik şikayetleri ile Sudan’da hastaneye başvurmuş. Parazitolojik incelemesinde P. falciparum tespit edilmiş. Meflokin ve artesunat tablet tedavi baş- lanmış, üç gün bu tedaviyi alarak Türkiye’ye dönen hasta, polikliniğimize başvurdu. Anemi ve trom- bositopeni tespit edildi. Hastaya doksisiklin 2x100 mg tablet tedavi başlandı. P. falciparum sıtması serebral malarya, hipoglisemi, anemi, trombositopeni ve pulmoner ödem gibi komplikasyonlarla seyredebileceğinden bu olguların yakın takibi gereklidir. Olgu; trombositopeni ve anemi yönünden izlenmiş ve ek müdahale gerekmeden şifa ile sonuçlanmıştır. Anahtar kelimeler: Plasmodium falciparum; Sıtma; Seyahat ABSTRACT Malaria is commonly detected in tropical and subtropical regions, P. vivax is the type generally seen in our country. However, P. falciparum malaria can also be seen among those who have a history of visiting abroad. Here, we reported a patient with P.falciparum whose treatment was started in Sudan due to suspicion of malaria, the diagnosis and follow-up was carried out in our clinic then after. Thirty-three years old male patient was admitted to outpatient clinic with complaints of fatigue and loss of appetite. In his history it was obtained that he had worked in Sudan and had applied to the hospital with complaints of high fever, sweating and weakness. In his parasitological examination, P. falciparum was detected. Mefloquine and artesunate oral treatment were started. After taking these medicines for three days, the patient returned to Turkey and applied to our department. Anemia and thrombocytopenia were detected. Doxycycline 100 mg twice a day treatment was started orally. Since P. falciparum related malaria clinically can cause complications such  as  cerebral  malaria,  hypoglycemia,  anemia,  thrombocytopenia  and  pulmonary  edema, close follow-up is necessary. In our case, the patient was put under follow-up for anemia and thrombocytopenia,  and he was recovered totally without needing any extra intervention.
